???metadata.dc.description.resumo???: Introdução: Grande parte dos estudos relacionados com a exposição aos pesticidas foi realizada com agricultores e não inclui o uso dos pesticidas na saúde pública e nem a exposição simultânea ao ruído. Por esse motivo, há necessidade em realizar estudos que verifiquem a ação ototóxica e neurotóxica da exposição simultânea aos pesticidas e ruído nos sistemas auditivo e vestibular dos servidores da saúde pública. Objetivo: Avaliar o impacto da exposição simultânea dos pesticidas usados na saúde pública e do ruído no sistema auditivo (periférico e central) e no sistema vestibular (periférico e central) de agentes de combate a endemias. Métodos: A amostra do estudo foi composta por agentes de combate a endemias expostos a pesticidas, do tipo inseticida organofosforados e piretróides sintéticos e ruído (grupo exposto), do sexo masculino, entre a faixa etária de 48 a 72 anos. Um grupo controle composto por 18 participantes, sem exposição a pesticidas e/ou ruído, foi utilizado para a comparação dos resultados, pareado com o grupo exposto conforme a faixa etária e sexo. Os participantes foram submetidos à entrevista fonoaudiológica, audiometria tonal em frequências convencionais e altas frequências, logoaudiometria, imitanciometria, pesquisa das emissões otoacústicas evocadas transientes e por produto de distorção, pesquisa do efeito de supressão das emissões otoacústicas transientes, potenciais evocados auditivos de tronco encefálico, teste dicótico de dígitos e avaliação vestibular. Resultados: Não houve diferença estatística entre os grupos na média dos limiares auditivos convencionais e de altas frequências, na logoaudiometria, na pesquisa das emissões otoacústicas evocadas transientes e por produto de distorção e no efeito de supressão das emissões otoacústicas transientes. A análise estatística evidenciou que a probabilidade de perda auditiva é 44 vezes mais no grupo exposto, independente do fator idade. Houve diferença estatística nos achados da timpanometria da orelha direita e no reflexo acústico, com maior número de casos alterados no grupo exposto. Nos potenciais evocados, houve diferença nas latências absolutas das ondas III e V e nas latências interpicos das ondas I-III e I-V, de ambas as orelhas, entre os grupos, com piores resultados no grupo exposto. No teste dicótico de dígitos, o resultado evidenciou maior ocorrência de casos alterados no grupo exposto, com associação significativa entre a exposição aos pesticidas e ruído e os resultados alterados (p=0,0099). Quanto aos resultados dos exames vestibulares, houve prevalência de exames alterados em 36,4% dos casos. Ao correlacionar os resultados da audiometria tonal com os exames vestibulares, observou-se que os participantes que apresentaram piores limiares auditivos na frequência de 4000 Hz da orelha direita também apresentaram resultados alterados no exame vestibular, sendo esta relação significante (p=0,0494). Conclusao: Conclui-se que houve impacto no sistema auditivo (periférico e central) e no sistema vestibular (periférico) dos agentes de combate a endemias expostos simultâneamente aos pesticidas e ruído. Efeitos no sistema auditivo foram observados na audiometria convencional, na timpanometria, no reflexo acústico, nas latências absolutas e latências interpicos dos potenciais evocados auditivos de tronco encefálico e no teste dicótico de dígitos. Além disso, a ocorrência de disfunção vestibular foi observada em mais de 1/3 da população estudada, sendo esta disfunção relacionada à alteração do labirinto anterior e posterior (Síndrome cócleo-vestibular).
Abstract: Introduction: Great part of studies related to pesticide exposure was held with farm workers and they include neither the use of pesticides in public health, nor the simultaneous exposure to noise. Therefore, it is necessary to carry out studies which investigate the ototoxic and neurotoxic effect of the simultaneous exposure to pesticides and noise on the peripheral and central auditory and peripheral and central vestibular systems of endemic disease control agents. Objective: To assess the impact of the simultaneous exposure to pesticides - used in public health - and noise on endemic disease control agents. Methods: The study sampling comprised endemic disease control agents, exposed to pesticides, synthetic organophosphate and pyrethroid insecticides, and noise (exposed group), males, age ranging from 48 to 72 years. A control group, comprising 18 participants, non-exposed to pesticides and/or noise, was used to compare results, paired with the EG, according to age range and gender. The participants underwent speech-language pathology interview, conventional pure tone and high-frequency audiometry, logoaudiometry, immittance testing, transient evoked and distortion-product otoacoustic emissions testing, suppression effect of the transient otoacoustic emissions, brainstem evoked response audiometry, dichotic digits test and vestibular assessment. Results: There was no statistical difference between the groups in mean conventional and high-frequency auditory thresholds, in the logoaudiometry, in the transient evoked otoacoustic emissions testing, in the distortion-product otoacoustic emissions testing, and in the suppression effect of the transient otoacoustic emissions. Statistical analysis evidenced that the odds ratio of hearing loss is 44 times higher in the exposed group, regardless of age. There was statistical difference in the findings of the tympanometry (right ear) and in the acoustic reflex, with greater number of abnormalities in the exposed group. In the evoked potential test, there was difference in the absolute latencies of waves I-III and I-V (both ears) between the groups, with worse results in the exposed group. In the dichotic digits test, the result evidenced greater occurrence of altered cases in the exposed group, with significant association between pesticide exposure and noise and altered results (p=0.0099). As for the vestibular screening, there was prevalence of abnormalities in 36.4% of the cases. By correlating the results of the pure-tone audiometry to vestibular screening, participants, who evidenced worse auditory thresholds at the frequency of 4000 Hz in the right ear, were observed abnormal results in the vestibular screening, thus, a significant correlation (p=0.0494). Conclusion: It can be concluded that there was impact on the peripheral and central auditory system and on the peripheral and central vestibular system of endemic disease control agents, simultaneously exposed to pesticides and noise. Effects on the auditory system were observed in the conventional pure-tone audiometry, in the tympanometry, in the acoustic reflex, in the absolute latencies and interpeak latencies of the brainstem evoked response audiometry and in the dichotic digits test. In addition, occurrence of vestibular disorder was observed in over 1/3 of the studied population, and that disorder is related to abnormalities in the anterior and posterior labyrinth (vestibulocochlear syndrome).
